Advance Care Planning (ACP) is a process for patients to plan for their future health and personal care. A discussion with patients and their loved ones to plan and document patients' wishes and healthcare goals. Should patients be unable to speak for themselves one day, the advance care plan will serve to guide patients' loved ones and healthcare team in caring for patients.
