Inpatient Services
Our inpatient services are nested in Alexandra Hospital Ward 5. This ward is designed to provide a healing environment for patients and families who require care with diagnosed advanced illnesses. Our goal is to help patients cope better and live well before leaving well.
A safe and comfortable environment is an important part of compassionate care. Our supportive care ward is designed to serve as a relaxing space. This serves to enhance the physical, emotional and spiritual well-being of its patients.
Close to nature: Views of lush greenery and plenty of natural light allows for a serene and relaxing surrounding.
Homely setting: Shared dining areas and spaces encourage social interaction and allow families to spend time together.
Privacy: As a patient approaches their last hours, quiet rooms with sufficient seating may be available for private moments.
Outpatient Clinic
Our clinic offers both physical and virtual visits. For selective cases, we can arrange a hybrid session, combining both clinic and home visit.
For virtual visits, we currently use Teleconsultation and google glass platforms. We will continue to explore new ways of reaching out to our patients.
Community Palliative Care via AlexNIC
Alex Nurses in Community (AlexNIC) is a community nursing programme that supports seamless transitions in care. Our AlexNIC (Palliative) nurses provide palliative care to patients in their homes.
We provide two main types of services: terminal discharge and transitional care.
Terminal Discharge for patients with prognosis of hours to days: Weekday and weekend support services
- Care giver training – Basic nursing care for end of life (skin & oral), administration and preparation of drug, wound care
- Facilitation and coordination care
- Initiation of infusion via disposable infusor
- Preparation of family for patient's last hours and death
- Psychosocial & spiritual support to patient and family
- Symptom assessment and management
- Bereavement support
Transitional care for patients with life limiting illnesses or prognosis with less than a year or with symptoms
- Weekday support
- Advance Care Planning at home (ACP@Home)
- Assessment and management of symptoms
- Blood taking
- Caregiver training
- Comprehensive assessment (Physical, Psychosocial, illness understanding and care preference, social economic resources, existential and spiritual concerns)
- Connection and coordination of care across settings
- Wound care management

AH Supportive and Palliative Care Program is one of the key pillars of services at Alexandra Hospital, that serve to help patients and family cope better when facing a life limiting illness.
