Alexandra Hospital's system of care is one which is imagined differently. Unlike conventional hospitals, departments which are based on the training backgrounds of the doctors will not exist in Alexandra Hospital.
Instead, programmes and services that are organised around the needs of the patients, have been created, according to the types of patients that the hospital will care for.
We have five key programmes - Well, Fast, Chronic, Healthy Ageing and Palliative Care. They will form the pillars of our clinical organisational structure. They will be supported by the surgical, rehabilitative, and urgent care services, with the aim of helping our patients feel Better.
The Well Programme aims to empower people to take charge of their own health, by being proactive with screening, exercise, and treatment, to prevent illnesses and Be Better.
The FAST Programme looks at emergency treatment from acute, sub-acute to rehabilitative care at our integrated wards.
With tailored care provided by the same care team under one roof, the programme seeks to make inpatient care a more hassle-free experience, and potentially shorten the length of stay, for patients to ultimately Get Better.
The
Chronic Programme, led by one doctor and one care team, consolidates a patient's multiple conditions and treatment plans, including medication and rehabilitation regime, into one single portfolio, cutting down on multiple appointments, helping patients
Live Better.
The Healthy Ageing Programme cares for the mind-related, mobility and sensory, orthopaedic, and other geriatric and social needs of the elderly in a holistic manner, so they can Age Better.
The Palliative Care Programme, coming in 2019, will provide compassionate care and quality of life, at every age and at every stage. It will serve to help patients Cope Better, when they live well before leaving well.
