Alexandra Hospital's (AH) Integrated General Hospital (IGH) model is a patient-centred approach designed to streamline care by consolidating multiple medical conditions under one principal doctor. Central to this model is the "firm" concept, where doctors from various specialties collaborate as a team, supporting the principal doctor behind the scenes. This reduces fragmentation, minimizes redundant clinic visits, and ensures holistic care, particularly for elderly patients with complex, multi-condition needs. From 2019 to 2025, over 90% of discharged patients saw only one principal doctor, demonstrating the model's success in enhancing efficiency and caregiver relief.
Our inpatient wards are designed to cater to patients with acute and urgent care needs. Under the IGH Model, each patient is managed by a dedicated Principal Doctor, known as the Generalist, and supported by a multidisciplinary One Care team. This team includes nurses, therapists, pharmacists, care coordinators, and medical social workers, all working together to provide holistic care. The Generalist consults with specialists as needed, ensuring a smooth transition from acute care to rehabilitation and discharge. Our goal is to streamline the patient's journey to wellness, minimizing transfers between facilities and care teams.
In our Medicine Specialist Outpatient Clinics, the IGH Model offers a one-stop care experience. Patients are primarily seen by a Generalist, who coordinates care and consults with specialists when necessary. This approach reduces fragmentation, providing patients with a single touchpoint for all their healthcare needs. The Generalist manages complex and multiple medical conditions, consolidating care to ensure it is delivered in a coordinated manner.
The IGH Model emphasizes the consolidation of care, reducing fragmentation, and ensuring that patients receive comprehensive treatment at different care intensity levels. The Generalist plays a dual role, providing general medical care while adopting a specialist role for conditions within their expertise. For conditions beyond their competency, the Generalist collaborates with specialists through transfer of care, shared care, or seeking specialist opinions without transferring care.
