About Integrated General Hospital

AH is one of the first Integrated General Hospitals (IGH) in Singapore. The IGH model is designed to provide patients with a coordinated and holistic care experience.

The IGH model is based on the following principles:

  • Patient-centred care: The IGH model puts the patient at the centre of care. Patients are assigned a single care team that is responsible for their care throughout their stay at the hospital. This team includes doctors, nurses, pharmacists, therapists, dietitians, social workers, and other healthcare professionals.
  • Integrated care: The IGH model integrates care across different settings, including the hospital, community, and home. This ensures that patients receive the right care at the right time, in the right setting.
  • Holistic care: The IGH model considers the physical, mental, and social needs of patients. This ensures that patients receive the full range of care they need to improve their health and well-being.

What is Alexandra Hospital IGH Care Model about?

Alexandra Hospital's (AH) Integrated General Hospital (IGH) model is a patient-centred approach designed to streamline care by consolidating multiple medical conditions under one principal doctor. Central to this model is the "firm" concept, where doctors from various specialties collaborate as a team, supporting the principal doctor behind the scenes. This reduces fragmentation, minimizes redundant clinic visits, and ensures holistic care, particularly for elderly patients with complex, multi-condition needs. From 2019 to 2025, over 90% of discharged patients saw only one principal doctor, demonstrating the model's success in enhancing efficiency and caregiver relief.

Inpatient Care with the IGH Model

Our inpatient wards are designed to cater to patients with acute and urgent care needs. Under the IGH Model, each patient is managed by a dedicated Principal Doctor, known as the Generalist, and supported by a multidisciplinary One Care team. This team includes nurses, therapists, pharmacists, care coordinators, and medical social workers, all working together to provide holistic care. The Generalist consults with specialists as needed, ensuring a smooth transition from acute care to rehabilitation and discharge. Our goal is to streamline the patient's journey to wellness, minimizing transfers between facilities and care teams.

Specialist Outpatient Clinics

In our Medicine Specialist Outpatient Clinics, the IGH Model offers a one-stop care experience. Patients are primarily seen by a Generalist, who coordinates care and consults with specialists when necessary. This approach reduces fragmentation, providing patients with a single touchpoint for all their healthcare needs. The Generalist manages complex and multiple medical conditions, consolidating care to ensure it is delivered in a coordinated manner.

Comprehensive and Coordinated Care

The IGH Model emphasizes the consolidation of care, reducing fragmentation, and ensuring that patients receive comprehensive treatment at different care intensity levels. The Generalist plays a dual role, providing general medical care while adopting a specialist role for conditions within their expertise. For conditions beyond their competency, the Generalist collaborates with specialists through transfer of care, shared care, or seeking specialist opinions without transferring care. At Alexandra Hospital, we are dedicated to delivering patient-centred care that is efficient, coordinated, and tailored to each individual's needs. Our IGH Model exemplifies our commitment to excellence in healthcare, ensuring that every patient receives the best possible care throughout their journey to recovery.

Integrated Medicine Programme
The Integrated Medicine Programme is the largest programme in Alexandra Hospital. It is anchored in Internal Medicine and driven by the IGH Model to provide patient-centred, holistic, and integrated care across inpatient and outpatient settings. The programme is supported by twelve medicine specialties and offers a suite of services such as virtual care, patient education, telemonitoring and many more.
Well Programme
The Well Programme aims to empower people to take charge of their own health, by being proactive with screening, exercise, and treatment, to prevent illnesses.
Healthy Ageing Programme

The Healthy Ageing Programme cares for the mind-related, mobility and sensory, orthopaedic, and other geriatric and social needs of the elderly in a holistic manner.

Supportive and Palliative Care Programme

The Supportive and Palliative Care Programme, will provide compassionate care and quality of life, at every age and at every stage. It will serve to help patients when they live well before leaving well.

Surgery & Anaesthesia Programme (Coming Soon)
The Surgery and Anaesthesia Programme is committed to providing an excellent perioperative journey through a multidisciplinary approach. Our team comprises nursing, anaesthesia and specialized surgical skill sets to provide surgical suites that offer high-quality, cost-effective surgery. 
Psychological Medicine Programme (Coming Soon)
The Psychological Medicine department commits to providing an integrated mental health service coverage across all age groups and care settings. In addition, programme and services aim towards building mental health capabilities within the community and advocacy of early intervention approaches. 
Rehabilitation Medicine Programme (Coming Soon)
The Rehabilitation Medicine department is dedicated to the rehabilitation and reintegrating of patients back into the community. The Rehabilitation Medicine department is committed to supporting our patients in their journey of recovery to gain independence and improve their quality of life through personalized and innovative rehabilitative care.
