Introducing the Chronic Programme (Live Better)

The Chronic Programme is focused on helping our patients with multiple complex co-morbidities Live Better.

It puts one doctor and one care team to manage the chronic conditions of each patient, reducing fragmentation and the need for multiple medications, as well as fewer specialist visits.

The Chronic Programme is served by the Integrated Care Clinic (I-Care Clinic), housed in Clinic J.
