Request for Medical Report

​For Medical Report requests, please apply via the NUHS App.

**Please add your child/parent as dependent in NUHS App if you are applying on their behalf.

Please follow the attached user guide to find out how to submit your medical report request(s) via the NUHS App.

Selective medical report fees applied via the NUHS App will be charged at a discounted price. Please refer to the app for more details.

Applying for a Medical Report

1.       For Singpass users: Please apply for a medical report via the NUHS App (under "Medical Report Request" module. You may download the NUHS App user guide for request medical reports here or watch the video tutorial here. Please refer to our cancellation policy prior to making your application.

2.       If you are applying on behalf of your family member (parent/ child), please add them as a dependent and apply under their profile.

Please submit an enquiry using this form if your request meets the following criteria:

a)       You are not a Singpass user

b)      You are applying on behalf of a patient who is unable to provide consent

c)       The medical report is not available on the NUHS App

d)      You have a payment enquiry regarding your request

e)       You are seeking a clarification on a previous medical report request

If you are requesting for a medical report type that is not available via the NUHS App, or are requesting for a medical report on behalf of another person, please complete Form A with one of the following form (s) (where applicable) with necessary supporting documents: 
The Patient / Child below 21 years old / Spouse / Parent Form A
Authorisation for Collection of Medical Report Form B
Deceased Patient Form C
Patient with Mental Incapacity
Form D
Foreign Worker who has left Singapore or is missing
Form E
Verification of Authenticity of Medical Documents (e.g. medical certificate, discharge summary) Form F

If you are requesting for medical report/records that is before June 2018, please request for the medical report/records through these respective administrations:

  • Khoo Teck Puat Hospital (Before Jul 2010) (click here). 
  • Jurong Health/ Ng Teng Fong General hospital (Aug 2010 – Aug 2015)  (click here). 
  • Sengkang General Hospital (Aug 2015 – May 2018) (click here).

Please be informed that any request for medical report will only be processed upon the receipt of the following items:

  • Completed signed application form
  • Required supporting documents
  • Payment of the medical report fees 

Cancellation Policy

There will be no refund provided upon cancellation of investigation results, duplication of medical certificate and discharge summary. 

For cancellation of all other reports, an admin fee of one-third the report price will be levied. No cancellation will be allowed once the report has been written by the doctor.
